2-Year Repair Service
We offer repair service for your FormStay bracelet for 2 years from delivery. Repair materials are provided at no charge. A $5 labor fee applies per bracelet, per repair.
Lifetime Name Additions
As your family grows and new people become part of your story, you can add their names to your existing FormStay bracelet.
Materials are included at no charge. A $5 labor fee applies per bracelet, per upgrade. Customers cover shipping both ways. We’ll confirm that your bracelet can accommodate the additional names before you send it.
How to Request Service

Email support@wearformstay.com with your order number, clear photos of your bracelet, and details of the repair or the exact names/text you would like to add.

We’ll confirm what is possible, the applicable labor fees, return shipping charges, and the address to send your bracelet.

Once you approve, pay the confirmed fees and send your bracelet using tracked shipping. We’ll complete the agreed work and ship it back to you.
Shipping Responsibility
Customers cover shipping in both directions: sending the bracelet to us and shipping it back after service. Return shipping costs will be confirmed before you proceed.
Our Engraving & Quality Promise
Engraving mistakes made by us, an incorrect size sent by us, or a bracelet that arrives defective remain covered by our separate Engraving & Quality Promise: a free replacement once confirmed, including replacement shipping.
